All 7 industrial permits have expired in September 2007. Currently being revised. Looks like Kentucky has an INDIVIDUAL permit for industrial activities and GPs for MS4s and construction.
From Individual permit-Kentucky does not have the permit language on website (although it might be elsewhere on the site), but they do have the Kentucky Pollutant Discharge Elimination System Permit Application (KPDES Form F). The following is taken from the instructions:
All samples shall be collected from the discharge resulting from a storm event that is greater than 0.1 inches and at least 72 hours from the previously measurable (greater than 0.1 inch rainfall) storm event. Where feasible, the variance in the duration of the event and the total rainfall of the event should not exceed 50 percent from the average or median rainfall event in that area. A grab sample shall be taken during the first thirty minutes of the discharge (or as soon thereafter as practicable), and a flow weighted composite shall be taken for the entire event or for the first three hours of the event.
